Data Breach Notice
October 24, 2025
FOR IMMEDIATE RELEASE
Data Security Incident
On September 19, 2025, Quality Podiatry Group (“Quality Podiatry”) was informed that their electronic health record provider, Modernizing Medicine, Inc. (“ModMed”), who hosts Quality Podiatry’s data, was involved in a security incident (“Incident”). ModMed assured Quality Podiatry they promptly launched an investigation and engaged a third-party forensics firm to determine the nature and scope of the incident. You can read more about the Incident here: https://response.idx.us/MMEvent/
ModMed’s investigation revealed that an unknown party accessed limited portions of Quality Podiatry’s data between July 9, 2025 and July 10, 2025. Per ModMed, impacted information may have included full name, address, date of birth, phone number, email address, health insurance information, and medical information (such as medical record number, patient account number, date(s) of service, provider and practice name, billing/diagnostic codes, prescription/medication information, and/or diagnosis and treatment information), and/or Social Security Numbers. They noted that not all data elements were involved for all patients.
While Quality Podiatry does not have any evidence that the information impacted by the Incident was used for fraudulent purposes, Quality Podiatry is unable to conclusively rule out that personal information was compromised. ModMed has continued monitoring the internet and other environments to determine if any of the impacted information has been used. As such, on October 30, 2025, ModMed will mail notification letters to the individuals potentially impacted by this incident. In those letters, ModMed offered identity theft protection services and credit monitoring through IDX for one year. Quality Podiatry encourages any affected individuals to remain vigilant by monitoring their personal information.
At Quality Podiatry, we take the responsibility of maintaining non-public patient information seriously, which is a major reason why we utilize the services of a professional provider like ModMed to host our patient data. ModMed has assured us they have, and will continue to, implement additional security controls to reduce the risk of a similar incident occurring again.
Quality Podiatry takes the privacy and the security of information in its possession very seriously and deeply regrets any inconvenience this incident may have caused.
